Every Measure, Every Facility
Measures in the FHI™ are coloured by their state: green = Met, yellow = Just short, red = Missed, navy = Not current. CMS-published measures that are not in the FHI™ are coloured against all U.S. facilities: green = top third, yellow = middle, red = bottom third.
Green is good and red needs work whichever way the measure runs — ↓ means lower is better, ↑ means higher is better.
★ means the measure counts toward a CMS star rating. ☆ means CMS publishes it but rates nothing on it. No star means CMS does not publish it — it is ours.
Health Inspection carries stars for the citation counts behind the inspection rating. Life Safety (K-tag) citations and plan-of-correction completion are published but sit outside that rating, so they carry an open star.
A flag beside a facility name means it is close to a line: ⚑ close to losing a star, ⚑ close to gaining one. Click the flag for the cushion.
The last column is the U.S. average for that measure.
